Mujer de 46 años que al año de edad presentó un pólipo perianal y máculas hiperpigmentadas en labios y mucosa oral (Fig1). Sin antecedentes oncológicos familiares. Posteriormente, ha presentado múltiples pólipos hamartomatosos en el tubo digestivo precisando resecciones múltiples. A los 32 años se diagnosticó de un papiloma intraductal y de un carcinoma ductal in situ, ambos en mama izquierda y se realizó mastectomía radical bilateral terapéutica y reductora de riesgo.
Ante la sospecha clínica de síndrome de Peutz-Jeghers (PJ), se realizó estudio genético dirigido del gen STK11, detectándose la variante patogénica germinal Q305X en heterocigosis, confirmando el diagnóstico clínico.
Durante la edad adulta en paciente con PJ, las recomendaciones de seguimiento son: colonoscopia, cápsula endoscópica, endoscopia alta, plantear ecoendoscopia y RMN pancreática, realización de RMN mamaria, mamografía anual y seguimiento ginecológico con ecografía transvaginal, Ca-125 y frotis endocervical.

A 46-year-old woman presented a perianal polyp and hyperpigmented macules on the lips and oral mucosa at one year of age (Fig1). No family cancer history. Subsequently, she has presented multiple hamartomatous polyps in the digestive tract, requiring multiple resections. At 32 years of age, she was diagnosed of an intraductal papilloma and a ductal carcinoma in situ, both in the left breast, and a risk-reducing and therapeutic bilateral radical mastectomy was performed.
Given the clinical suspicion of Peutz-Jeghers (PJ) syndrome, a directed genetic study of the STK11 gene was performed, detecting the pathogenic germline variant Q305X in heterozygosis, confirming the clinical diagnosis.
During adulthood in a patient with PJ, the follow-up recommendations are: colonoscopy, capsule endoscopy, upper endoscopy, consider pancreatic endoscopic ultrasound and MRI, mammary MRI, annual mammography and gynecological follow-up with transvaginal ultrasound, Ca-125 and endocervical Pap smear.
