Varón de 75 años diagnosticado en mayo de 2015 de osteosarcoma de alto grado en tercio proximal de humero derecho, resecado con márgenes libres. En septiembre de 2015, presentó recidiva local tratada mediante amputación reglada de miembro superior derecho y quimioterapia adyuvante con cisplatino y adriamicina.
En septiembre de 2018, se constató recaída ósea a nivel de L2 que presentaba el característico, aunque infrecuente, signo “en sol naciente” (Figura 1: Imagen en TAC, corte axial y Figura 2: Imagen en RMN-T2, corte sagital). El paciente recibió tratamiento con radioterapia paliativa sobre L1-L2-L3, dosis total: 30 Gy.
A pesar de ser una localización infrecuente, la historia oncológica y la patogénesis de este signo en “sol naciente” con rotura de hueso cortical y extensión a tejidos blandos tras elevar el periostio, favorecieron el diagnóstico de metástasis ósea secundaria a osteosarcoma.

A 75-year-old man was diagnosed with a high-grade osteosarcoma in the proximal third of the right humerus, resected with free margins in May 2015. In September 2015, he suffered local recurrence and was treated with amputation of the right upper limb and adjuvant chemotherapy with cisplatin and adriamycin.
In September 2018, bone relapse was found at L2 level with the characteristic, although infrequent, sign "in sunburst " (Figure 1: CT image, axial section and Figure 2: MRI-T2 image, sagittal section). He was treated with palliative radiotherapy on L1-L2-L3, total dose: 30 Gy. Despite being an infrequent location, the oncological history and pathogenesis of this sign with cortical bone rupture and extension to soft tissues after raising the periosteum, causing reactive bone growing in perpendicular sheets and giving the appearance of a “sunburst” facilitated the diagnosis.
